Note : In my opinion criminally underseen, this nightmarish modern take on Lovecraftian cosmic horror is fairly low-budget, but manages to do a whole lot with what it has, including stylish cinematography, decent acting, gnarly practical creature and gore effects, and a pretty good story, with a third act that feels very similar to the ending of Fulci's The Beyond, while the whole thing evokes at least a couple of different Carpenter movies. If that sounds like your kind of thing, you will not be disappointed. As a bonus, look out for a small role from Canadian horror stalwart Art Hindle.

A cop comes across a wounded man on a road in the middle of the woods, and brings him to the hospital where his ex-wife works, which turns out to be very shortstaffed and largely abandoned following a fire. Soon after his arrival, things get strange, with a nurse murdering a patient after flaying her own face, the cop shooting her, and then experiencing a seizure and visions, and a state trooper arriving to take the wounded man into custody. A group of robed and hooded cultists armed with knives surround the hospital, and two men who are hunting the wounded man from the woods arrive looking for him. The dead nurse transforms into a horrible, tentacled creature, and tensions rise as the trapped group tries to stay alive and figure out what the hell is going on.

As with most modern films, this Blu-Ray looks basically perfect, and the low levels of grain and noise made it a pretty simple encode. 5.1 track is also fine, and the directors' commentary very worth listening to.
